Missing records of a hospital visit or even documents from another patient. There appears to be a lot wrong with medical records.
Errors in files
After several complaints, the Consumers' Association sampled 50 requested medical files. In two-thirds of the files, parts were incorrect or incomplete. Sometimes whole years are missing and in two files an antibiotic allergy was missing.
No file available
In eight cases it was even impossible to receive the file, while patients have the right to inspect or a copy.
Advice
The Consumers' Association advises requesting and checking your own file. In addition, general practitioners and pharmacists must monitor their data more closely.
